Person Centred Plans

We have developed our SRS Creative Futures team to focus solely on person centred, family sensitive planning and to remain separate from other SRS services whose core responsibilities centre around the delivery of supports.

Most requests for planning support are to assist people to live in their own homes or move into alternate accommodation settings.  Other requests include (but are not limited to):

• Leaving school
• Supporting families to maintain their caring role
• When carers are no longer able to care for their loved one
• Getting a job
• Managing complex health needs
• Moving interstate
• Getting a drivers licence

The other main function of SRS Creative Futures is to be the contact point for all new enquiries and requests for support from SRS.  This intake role allows the SRS Creative Futures team to plan with families how they would like their support to look and explore other areas of assistance that may be available.
